The Significance of Vitamin D

Vitamin D is a fat-soluble vitamin that is essential for maintaining immune system function, mood regulation, and disease prevention.

Raising vitamin D levels isn’t an overnight endeavor. Typically, for someone deficient in Vitamin D, it could take months of regular supplementation to achieve blood levels above 50 ng/mL, a level often cited as optimal for overall health. However, its effectiveness is evident, as backed by many studies:

• A study from 2021 showed that 66% of subjects without skin cancer had vitamin D deficits, while only 34% with these types of skin cancers exhibited similar deficiencies.
• A meta-analysis from 2023 showed that higher Vitamin D intake and serum 25(OH)D levels were significantly linked to a reduced risk of cancer and a decrease in cancer-related mortality.
• In a systematic review and meta-analysis, it was found that COVID-19 mortality risk inversely correlates with Vitamin D3 status, suggesting that a mortality rate close to zero could theoretically be achieved at 50 ng/mL 25(OH)D3.

The 50 ng/mL Benchmark

There’s been a surge in literature hinting that individuals with vitamin D blood levels of 50 ng/mL or above generally maintain better health. This is backed by a study that showed that people who still living the lifestyle we evolved in, like the Maasai and Hadzabe, have average serum 25(OH)D concentrations of 119 nmol/l (47.6 ng/mL) and 109 nmol/l (43.6 ng/mL) respectively. Notably, individuals with Vitamin D bloodserum levels of higher than 50 ng/mL seem to exhibit resilience against many ailments, especially respiratory challenges. The mantra seems to be clear: adequate vitamin D could be a shield against several health adversities.

Enter Calcifediol

While Vitamin D supplementation, specifically Vitamin D3 cholecalciferol, is widely recognized and either ingested or produced in the skin via the action of UV-B light, there’s an alternative that’s gaining traction: calcifediol.

Calcifediol, also known as calcidiol, is a prehormone metabolized from Vitamin D3, predominantly in the liver. Its distinction lies in its bioavailability. Unlike regular Vitamin D3 in the form of cholecalciferol, calcifediol is highly bioavailable, meaning the body can use it more directly and effectively.

This efficacy allows calcifediol to rapidly raise vitamin D blood levels, achieving results in mere hours instead of the months typically associated with Vitamin D3 supplementation. This property proves invaluable, especially in situations where an accelerated immune response is desired. Regarding dosages, they often vary based on individual factors, but a common guideline is in the realm of milligrams per kilogram of body weight to quickly hit that desired 50 ng/mL benchmark.

Raising Calcifediol Levels in Clinical Emergencies

Although 5,000 IU may seem like a substantial amount of vitamin D3, this minuscule 1/8000th of a gram does little in a single day to elevate unsupplemented average 25-hydroxyvitamin D levels, which typically range from 5 to 25 ng/mL. A common method for rapidly increasing 25-hydroxyvitamin D levels is an oral bolus (or “loading”) dose of vitamin D3.

According to the European Society for Clinical Nutrition and Metabolism (ESPEN) Guideline on clinical nutrition in the intensive care unit, a single high dose of 500,000 IU (12.5 mg) of vitamin D3 appears safe for patients with deficiency during the first week of treatment, especially when taken in conjunction with Vitamin K2-MK7. However, in critically ill patients, potential limitations on absorption and on liver function, and the four or so days it takes to hydroxylate the vitamin D3 into 25-hydroxyvitamin D, unacceptably delay the urgently needed improvement in immune function.

Dosing of Calcifediol

A more effective approach involves a single oral dose of calcifediol (25-hydroxyvitamin D) of 0.014 mg per kilogram body weight. For an average weight adult, this is approximately 1 milligram. Calcifediol is more readily absorbed than vitamin D3 and directly enters circulation, safely elevating levels above 50 ng/mL within just 4 hours. This boosted level decreases over days to a week, so further calcifediol or regular or bolus vitamin D3 supplementation is necessary to maintain it. A 1 mg dose of calcifediol is roughly equivalent to a modest 160,000 IU (4 mg) of vitamin D3. Unless there is reason to suspect the patient has excessive 25-hydroxyvitamin D, blood tests are unnecessary, as this intake does not lead to toxicity.

In patients hospitalized with COVID-19, for instance, calcifediol treatment notably decreased the need for ICU admission and slashed the risk of death by 79.0% (RR 0.21, p = 0.001).

Consistency is Key

Many peer-reviewed research articles conclusively demonstrate that a low level of 25-hydroxyvitamin D in the bloodstream significantly increases the risk of severe disease, long-term damage, and death from COVID-19. In most populations, average 25-hydroxyvitamin D levels are half or less than the 50 ng/mL (125 nmol/L) required for optimal immune system function. The lack of sun exposure or vitamin D supplementation can be one factor, but what many people do not know is that the intake of magnesium is also crucial.

The Invisible Deficiency: Magnesium and the Western World

A surprising number of people are deficient in magnesium, an element vital for our well-being. But why is this a problem? Because magnesium is a key player in the metabolic pathway that facilitates Vitamin D synthesis. This is attributable to the fact that magnesium serves as a cofactor in over 300 enzymatic reactions, which are crucial for the structural function of proteins, nucleic acids, and mitochondria, as well as for the synthesis of Vitamin D. Consequently, if magnesium is scarce, Vitamin D can’t fulfill its critical roles in bone health and immune function. This is an issue that could rewrite the narrative on public health.

“Vitamin D” blood tests measure the level (concentration) of 25-hydroxyvitamin D in the bloodstream, because both the kidneys and the immune system rely on this as a supply for their signaling functions, which involve hydroxylating it to 1,25-dihydroxyvitamin D. 25-hydroxyvitamin D is made primarily in the liver, from vitamin D3 which is either ingested or is produced by the action of short-wavelength, high energy, ultraviolet B radiation on the skin.

In most populations, average 25-hydroxyvitamin D levels are half or less than the 50 ng/mL (125 nmol/L) required for optimal immune system function. There is very little vitamin D3 in food. UV-B radiation is difficult to obtain except on cloudless days with high elevation sunlight – and it always damages DNA and so raises the risk of skin cancer.

Good health can only be obtained with at least 50 ng/mL circulating 25-hydroxyvitamin D. This is the only way of ensuring that SARS-CoV-2 and influenza do not spread in pandemic fashion and do not seriously harm and kill many of those who are infected. Sepsis, which kills around 11 million people a year worldwide, would be rare if everyone had at least 50 ng/mL circulating 25-hydroxyvitamin D.

Attaining 50 ng/mL 25-hydroxyvitamin D

Food can provide only a small fraction of the vitamin D3 we need to maintain good health. UV-B skin exposure is neither a safe nor easily accessible method for generating most or all of the vitamin D3 our bodies require. Proper vitamin D3 supplementation is the only way for most people to achieve the 25-hydroxyvitamin D levels their immune systems need to function properly. The daily quantities are minimal; for convenience and cost reduction, larger doses can be taken every 7 to 10 days. Current government-approved vitamin D3 supplemental guidelines are insufficient. They aim to achieve a circulating 25-hydroxyvitamin D level of just 20 ng/mL, which is generally adequate only for kidney function and bone health. These guidelines typically specify supplemental amounts based on age group, often with a maximum daily intake of 0.1 to 0.25 mg (4,000 to 10,000 IU).

To achieve healthy 25-hydroxyvitamin D levels for all individuals, we cannot rely solely on these recommendations, blood tests, or medical monitoring. Fortunately, there is a relatively wide range of healthy vitamin D3 intakes for each person, owing to the self-limiting actions of enzymes that degrade 25-hydroxyvitamin D at a rate proportional to its level in the body. As a result, doubling the vitamin D3 intake is likely to increase 25-hydroxyvitamin D levels by only 40 to 50%.

In order to achieve healthy 25-hydroxyvitamin D levels of at least 50 ng/mL, and up to 90 or 100 ng/mL without medical monitoring, it is both necessary and sufficient to specify the average daily vitamin D3 supplemental intake as a ratio of body weight, or within a range of two ratios, with higher ratios for those suffering from obesity. Ideally, leading vitamin D researchers would already have written a consensus journal article to this effect. Recommendations consistent with this approach can be found in a 2022 Nutrients article by Emeritus Professor of Medicine Sunil Wimalawansa (University of Texas Medical Branch at Galveston and Robert Wood Johnson Medical School, New Jersey): “Rapidly Increasing Serum 25(OH)D Boosts the Immune System, against Infections – Sepsis and COVID-19.” These recommendations include the following ranges of ratios:

• Normal and overweight: 60 to 90 IU per kilogram body weight, per day. Thus, 0.125 mg (5,000 IU) daily, or a 50,000 IU capsule every 10 days, is suitable for individuals weighing 56 to 83 kg (122 to 183 lbs).
• Obesity I & II (BMI 30 to 39): 90 to 130 IU per kg body weight per day.
• Obesity III (BMI 40 or more) – a condition requiring medical attention: 140 to 180 IU per kg body weight per day.

Ratios of bodyweight for underweight, normal and overweight people, with higher ratios for those suffering from obesity, can be derived from Afshar et al. 2020 who found that 70 to 100 IU vitamin D3 a day per kg bodyweight, for over 500 neuro-opthamology patients since 2010, resulted in long-term 25-hydroxyvitamin D levels between 40 and 90 ng/mL. In general, those at the lower end of this range would have been suffering from obesity.

Ekwaru et al. 2014 analyzed long-term 25-hydroxyvitamin D levels according to daily supplemental intake quantities for four self-described body morphologies: underweight, normal, overweight and obesity. From this it is possible to conclude that in order to attain 50 ng/mL 25-hydroxyvitamin D, those suffering from obesity needed approximately 43% more vitamin D per day, as a ratio of bodyweight, than those not so suffering.

Prof. Wimalawansa adapted this derivation to the ranges of ratios just mentioned, which we summarize in a simplified manner in the following table:

He also recommended 70 IU vitamin D3 / kg BW / day for age 18 years and younger (with no correction for obesity) and lower ratios for underweight adults.

Research indicates that two mechanisms cause people suffering from obesity to attain lower 25-hydroxyvitamin D levels than those without obesity, for any given ratio of vitamin D3 supplemental intake and body weight: reduced hydroxylation of vitamin D3 in the liver and increased absorption of 25-hydroxyvitamin D in excess adipose tissue. These mechanisms are specific to obesity. We know of no mechanistic or observational evidence for lower ratios for underweight people.

By following body weight ratio based recommendations such as those made by Prof. Wimalawansa, people of all ages, weights and body morphologies will attain at least 50 ng/mL of circulating 25-hydroxyvitamin D over several months, with levels rarely exceeding 100 ng/mL, without the need for blood tests or medical supervision.

Vitamin D deficiency pandemic

There is no flu season, just low vitamin D season. About 80% of US Americans suffer from vitamin D deficieny. Skin needs to be struck by sunlight to synthesize vitamin D, an immunoregulating hormone. Dark skin contains melanin, an effective natural sunscreen that reduces vitamin D synthesis. The correlation of different skin colours worldwide is perfect with UVA/UVB radiation intensities. Neither cancer nor God or diversity had anything to do with the evolutionary pressure on the skin pigments. It’s simply a health feature!

The following illustration is derived from a paper that determined the vitamin D status among UK biobank participants. It shows that the darker the skin colour, the higher the likelihood of vitamin D deficiency.

Hardly anyone in that survey achieved vitamin D levels above 30ng/mL. In addition, a publication from 2020 shows a strong correlation between vitamin D bloodserum levels and the severity of COVID.

“25(OH)D deficiency was observed among patients with COVID-19. Declined steadily 25(OH)D levels make a huge contribution to the scale of the progression of the disease.”

My diagram below illustrates the paper’s findings and shows that levels above 40ng/mL are more favourable. However, proper immune system function requires at least 50ng/mL, as my buddy Robin Whittle explained in this article.

This assumption is supported by Dror et al. (2021). They state that “patients with vitamin D deficiency (<20 ng/mL) were 14 times more likely to have [a] severe or critical [course of the] disease than patients with 25(OH)D ≥ 40 ng/mL”.

Katz et al. (2020) made similar observations. In their paper, they stated:

1. Vitamin D deficiency is strongly associated with an increased risk for coronavirus disease 2019 (COVID-19).
2. The odds ratio for COVID-19 increases with vitamin deficiency in black individuals.
3. Diabetes, obesity, and periodontal disease are associated with an increased risk for COVID-19 and vitamin D deficiency.

The website vdmeta.com lists all available studies concerning vitamin D and Covid. The evidence that vitamin D prevents getting sick from Covid is overwhelming. However, many researchers administered vitamin D in the form of cholecalciferol, which takes weeks to raise the vitamin D bloodserum level. Instead, calcifediol (i.e. 25(OH)D should have been administered, as 1mg of calcifediol raises average levels from 20 to over 60ng/mL in less than 4 hours. This is the treatment all intensive care patients require but won’t get.

Obesity

Obesity and vitamin D deficiency go hand in hand. The reasons for this are as follows:

• The higher the degree of obesity, the higher the amount of blood circulating. More blood requires more vitamin D to achieve favourable levels.
• Obese people usually go outside less. Also, many are ashamed to show skin in public.

However, vitamin D deficiency does not cause obesity. Instead, the root causes lay somewhere else: the diet.

About 75% of US Americans are overweight or obese. What these people generally have in common is that they consume:

1. Processed foodstuff
2. Polyunsaturated vegetable oils rich in Omega-6 fatty acids
3. High amounts of sugars (Glucose, sucrose, fructose)
4. Way too many carbs

A meta-analysis shows that being obese was significantly associated with more severe disease and mortality. The reasons for this are the above-mentioned vitamin D deficiency, higher inflammation levels, and a generally poorer intestinal flora (>70% of the immune system is located in the intestine).

It is a mystery why governments and the media have admitted for over two years that obesity is the main comorbidity, and almost exclusively obese and elderly people died with a positive PCR test. But they have done absolutely nothing to help people lose weight and live healthier lives. Instead, sports clubs were closed, people were told to stay indoors (less movement and less sun exposure), and experimental and very likely harmful gene therapies were promoted. Nobody, young and healthy, must fear a seasonal respiratory virus. If the ruling authorities really cared about our health, they would have tried to fix the two real pandemics: vitamin D deficiency and obesity.

It’s up to us to be healthy!

1. Get out in the sun and supplement with vitamin D and K2 (to avoid calcification). Ensure your vitamin D blood serum level is above 50ng/mL.
2. Glutathione is the mother of all antioxidants. Eat pasture-raised eggs and selenium-rich foods to boost your glutathione levels. We need selenium to power glutathione peroxidase (GPX), the enzyme that removes free radicals inside the cell. Also, eat foods that contain B vitamins as they power glutathione reductase, an enzyme that recycles glutathione.
3. Consume fatty sea fish as they are rich in DHA and EPA (Omega-3 fatty acids) at least once a week. DHA is critical as it (1) lowers the genetic expression of four types of pro-inflammatory proteins and (2) lowers white blood cell secretion of three types of pro-inflammatory proteins.
4. Exercise several times a week. Exercising helps with weight loss, reduces your risk of chronic disease, and can help you sleep better.
5. Give up on sugars, vegetable oils, and (ultra-)processed foods. Cook with ghee, butter, extra-virgin olive oil, or cocos oil instead.
